Chronic Care Management
Chronic Care Management (CCM) is a critical component of primary care that contributes to better health and care for individuals. CCM is care coordination services done outside of a regular office visit for patients with two or more chronic conditions expected to last at least 12 months or until the death of the patient, and that place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline.
Our clinically trained pharmacists partner with local primary care providers to make sure our patients receive the best overall that we can provide.
Transitions of Care
Our pharmacists specialize in care transitions, or the movement of a patient from one setting of care to another. We coordinate with physicians, nurses, and others as part of your entire health care team to monitor your continuing health, provide bedside delivery of medications at discharge, and to set the overall goal of quality of life for transitional patients.
